Easy Pay is a payment option for the convenience of our clients who seek to provide the best for their animals health and comfort, the Clinic will accept checks on the day of service for deposit at a later prearranged date.
Example: If your balance is $300 dollars then your deposit on the day of service would be $90. The remaining balance would be due in 3 equal monthly payments of $72.11. A credit card number would be recorded as a guarantee of payment.
Terms and conditions:
1. 30% of the total bill must be paid prior to discharge of the animal.
2. The balance may be divided into no more than 3 equal payments.
3. The first check must be deposited within 30 days of the date of service.
4. The total balance must be paid within 3 months of the date of service.
5. I agree to pay a monthly billing and financing fee equal to 1.5% of any unpaid balance.
6. There will be a $20 accounting fee for EACH check dishonored by the client's bank or held 10 days or more past the date scheduled for deposit of that check for which services were rendered, regardless of the amount.
7. In the event of default your credit card will be used to ensure payment. Should it become necessary for the practice to collect the unpaid amount through an attorney or collection agency, I agree that an amount equal to 50% of the total bill will be added to cover collection agency fees.
